If you decide to terminate a pregnancy, it is important to understand your options which include a D&C or procedural abortion (offered at our clinics up to 20 weeks in gestation) or an Abortion Pill (FDA Approved and offered at our clinics up to 11 weeks in gestation). A simple ultrasound determines the gestation.

Presurgical Teaching: Procedure Abortion (Aspiration / D&C)

A procedure abortion is a safe and common procedure used to end a pregnancy. It is typically done in the first trimester (up to 13 weeks) or second trimester (up to 20 weeks, depending on location and laws).

✅ Medical Evaluation

  • Pregnancy test to confirm pregnancy
  • Ultrasound to confirm gestational age
  • Blood tests: Rh factor, hemoglobin (if your pregnancy over 12 weeks or you have a history of anemia)
  • Informed consent: you will review the procedure and sign a consent form

🥼 You may also receive:

  • Antibiotics to prevent infection
  • Pain medication or sedation (oral, IV, or local anesthesia)
  • Medications to dilate the cervix, especially if you're in the second trimester

First Trimester: Manual or Vacuum Aspiration

  • A speculum is inserted into the vagina
  • The cervix is gently dilated (if needed)
  • A suction device removes the pregnancy tissue
  • Procedure takes about 5–10 minutes

Second Trimester: Dilation and Evacuation (D&E)

  • May require cervical preparation that may take 2-4 hours
  • Similar steps, but instruments may be used to assist with removal
  • Procedure may take 10–30 minutes

Options may include:

  • Local anesthesia (numbing the cervix, you are awake during the procedure)
  • Moderate sedation (You remain arousable but may have limited memory of the procedure, with the goal of providing comfort, pain relief, and anxiety reduction during certain medical procedures).

You will discuss what’s appropriate and safe based on your health and gestational age.

Normal symptoms:

  • Cramping (similar to period cramps)
  • Vaginal bleeding for up to 2 weeks (light to moderate)
  • Emotional reactions: relief, sadness, or mixed feelings

Recovery:

  • Rest for the remainder of the day
  • Most return to work/school in 1–2 days
  • Avoid tampons, douching, and sex for at least 1 week (or as advised)

Call your provider or go to the ER if you have:

  • Heavy bleeding (soaking >2 pads/hour for 2 hours)
  • Fever over 100.4°F (38°C)
  • Severe abdominal pain not relieved by medication
  • Foul-smelling vaginal discharge
  • You can become pregnant again within 2–3 weeks
  • Discuss contraceptive options before or after the procedure
  • A follow-up visit may be scheduled in 3-4 weeks to ensure recovery
  • It’s normal to have a range of emotions
  • Consider talking to a counselor or support group if needed
  • Your provider can give you mental health or community resources
  • Bring a support person if possible
  • Wear comfortable clothes
  • Do not eat or drink after midnight if you're receiving sedation
  • Arrange for transportation home if you’re sedated. Remember, you cannot drive after you receive sedation. You need someone to drive you home. Your safety is our priority.
